I, the parent or guardian of hereby give permission for my child (hereinafter “child” (*2)) to attend and to participate in Memorial Weekend Kids Camp on May 26-28, 2018 (hereinafter, the”camp”) at Frost-valley YMCA (2000 Frost Valley Rd, Claryville, NY 12725) sponsored by BJAFA and Tokyo YMCA.
I, on behalf of myself, my child, my family and any invited guest I may bring to the camp, waive, release, hold harmless, and discharge BJAFA, and their respective directors, officers, board members, employees, volunteers, agents, representatives or assigns, including the camp sponsors, from any and all liability, including but not limited to, liability arising from negligence or fault of the entities or persons released, for bodily injury, including a reaction to or sickness resulting from the ingestion of food or beverages, or liability for property damage or theft of any kind with respect to property at the camp. It is further understood that all participants in the camp assume all of the risks of participating and/or volunteering in the camp. I, my child, my family and any invited guest I bring to the camp agree to hold harmless and indemnify BJAFA, and their respective directors, officers, board members, employees, volunteers, agents, representatives or assigns, including the camp sponsors, and promise not to sue the entities or persons named above with respect to any liabilities, claims, demands or otherwise made or arising from or related to participation in this camp, whether caused by the negligence of those entities or persons released or otherwise.
I further recognize that by signing this Participant Consent and Release Form that the likeness or image of myself, my child, my family or any invited guest I may bring to the camp may be captured by camera or video device and I hereby release all rights to and uses of said likenesses or images to BJAFA so that BJAFA may publish and utilize said images taken during the camp.
This consent and release form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
